Moby Focus Group Participant Waiver
1. Consent to Participate
I (“Participant”), voluntarily agree to participate in the Moby Focus Group (“Focus Group”) hosted by Moby Wrap LLC. I understand that participation is voluntary and may involve open discussion in a group setting.
2. Photo, Video, and Audio Recording Release
I understand that the Focus Group may be recorded through photographs, video, and audio. By signing this form, I hereby grant Moby Wrap LLC, its representatives, and affiliates:

Permission to record, photograph, and/or film my participation in the Focus Group.

Full rights to use, reproduce, publish, distribute, and otherwise make use of these recordings or images for internal, promotional, educational, research, and commercial purposes.

Waiver of any right to inspect or approve the finished product or to receive any compensation.
This release applies to me and, if applicable, my child or dependent attending the Focus Group with me.
3. Liability Waiver and Release
I acknowledge that participation in the Focus Group involves certain risks, including minor injuries or property loss. In consideration of being permitted to participate, I hereby:

Release, waive, and discharge Moby Wrap LLC, its officers, employees, agents, representatives from any and all liability, claims, or demands arising from loss, theft, or damage to personal property; or injury, illness, or harm to myself or my child/dependent during or associated with the Focus Group.

Agree to assume full responsibility for any risks of participation.
This waiver is intended to be as broad and inclusive as permitted by law and shall be governed by the laws of the State of Tennessee.
4. Confidentiality Agreement
I understand that all discussions, materials, feedback, and observations shared during the Focus Group (“Confidential Information”) are proprietary to Moby Wrap LLC. I:

Agree not to disclose, share, copy, or distribute any Confidential Information to any other person, company, or entity without written consent from Moby Wrap LLC.

Acknowledge that this confidentiality obligation applies both during and after the Focus Group.
